Header row is case-insensitive. Extra columns are ignored.

Snappys / Eby-Brown — required: group, section, cost, description, item. Optional: pk_srp, gp, um, guar, cspk.

Core-Mark Cig — required: section, name, item. Optional: size, rank, build_to.

For both templates, item is the value encoded as the Code 128 barcode. Weekly fill-in cells are left blank on the PDF for hand entry, exactly like the printed guides.
